As a not-for-profit social impact services platform, we support human services organizations in digitizing service delivery through the use of technology, so that case workers can prioritize their services to improve connection, safety, prevention, and protection of vulnerable people while minimizing risk.
Digital Case Connect is committed to developing deep and meaningful client experiences during the implementation of human services case management solution. We give social workers the chance to learn how to use digital tools like Primero in their work with individuals, groups, and communities. Our members contribute to and learn from the various aspects of social work, as well as how social workers effectively address casework challenges. Some of these are policies, guidelines for practice, technology tools, easy processes, education, and training.
We invest in our relationships so that we can offer flexible, tailor made solutions. We focus on engagement, innovation and minimization of complexities using digitization. We support setting up the essential program or services delivery infrastructure necessary for social services organizations to more easily and effectively carry out their mission.
Case management is the process of planning, managing and delivering the actions and decisions required to achieve the objective or outcome required by the case throughout its life cycle. The subject of a case is a citizen. The needs of the individual and family constitute the case. Thus, a case contains the evolving set of information about the client and the objective required to make the decisions and drive the actions required to achieve the objective(s).
Cases are typically opened when the objective or outcome is identified and closed when this is achieved. Cases may be re-opened. A case is typically opened or closed based on the life cycle of the desired objective or achieved outcome, which may or may not be the same as that of its subject.
Human services case management significantly augments case management and is defined as a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive financial, health, and social well-being.
